How do you set up a document where an eSignature needs to tick an endorsed/not endorsed check box?

Hi,

Thank you for reaching out here in the DocuSign Community. 

If checkboxes are not a good option for your use case, you might want to look into using radio buttons instead.

And, in case you are looking for your recipient to approve the document, instead of signing it, you can use our approve button.
