I have downloaded the template but am wondering if there is a way to have the form fillable for SS# instead of EIN#

The W9 template provided by DocuSign has a fillable field for both SSN and EIN, if you would like the document to only show SSN as an option, you can remove the fields that are for the EIN (radio button and text fields).

You can always create a case to have a Support agent work the case with you.

If you are not able to open the case on the same page, you should scroll down to More Support Options and select "I can't reset my password or don't have an account." you will have the option of filling out the form or calling Support.

Thank You so much. Is there a way to have an option to put the business name if different then the line 1 name?

Yes, you can drag and drop a text field to the needed place and change the formatting so that it can match the other fields (formatting meaning the letter size or font) you can also set it to optional, by default it is set to required. All this can be done on the right menu when selecting the field.
