To help our customers automate recurring tasks, DocuSign eSignature offers the option to create templates of your most used documents and forms. 

Any user with Template permissions will be able to create templates.

in at your company and ask that your User Profile be updated. 

In the case that you would like the fields to be added automatically when you upload a document, you can look into coupling templates with our Template Matching feature.
