To provide uninterrupted service, your monthly or annual Docusign subscription renews automatically on your billing anniversary date to the latest equivalent plan.
To check when it’s your billing anniversary date, please access Settings > Plan and Billing, and check your last invoice date issue.

I had the same problem and VR’s response doesn’t really address the issue - the limit is 5 sends a month, not annually.
Does the 5 sends renew on a calendar month basis or on a monthly basis based off the subscription date?
In either event, I should not show 0 sends left . . . is docusign just slow to update month to month?
Inquiring minds want to know!! :-)

Docusign subscription renews automatically on your billing anniversary date. In other words, if you purchased the plan on the 10th, your plan will only be renewed on the 10th of the following month.
To check when it’s your billing anniversary date, please access Settings > Plan and Billing, and check your last invoice date issue.
