Yes or No

Question

I have a document that has several “Yes or No” questions and I don’t know how to make all of the boxes independent of each other? It seems when one box is selected then all of the boxes become selected with the same answer.

@ProHealthONeCheck the Data Label on your Field, if using the same Data Label on multiple checkbox or radio button groups that will cause what is called Data Replication. Make sure each Data Label is unique.
