require either yes or no as an answer to questions

I’m new and creating a template from an existing Word document and it’s going fine.  But, I’m at a point in the template that REQUIRES either a yes or no answer to several questions (this is a background check).  How do i set it up that the applicant must answer either yes or no?

Videos are good - I just don’t know how to ask the question to get the video.

You can either use radio buttons or a required dropdown with the values “Yes” and “No”. Depending on how your document looks, the one or the other solution works better for you.
