Does this happen when creating a new envelope by using the template by itself?
If that is the case, then the root cause of the issue must be related to the fields in your template and not the PowerForm. Under these circumstances I would suggest deleting each field individually to confirm which one is causing the error, or has a duplicate field saved under it. The scenario in which there might be duplicate fields under your existing fields is quite common, and could also cause this error.
