Have you tried to set up a conditional field with the Radio button? How do I set a conditional field based on the response to one or more radio button groups?

Conditional fields allow you to create dynamic documents that show specific fields to the recipient based on certain conditions being met, such as selecting a checkbox or dropdown option.
