Hello All,
On Friday we sent a document as normal (we use the same template and process every month) and during the testing everything was fine. After we had selected send we received reports from those in receipt that an additional document was also attached to the email they received which shouldn’t have been. Did anyone else experience this?

You are welcome to the Docusign Community!
Are you talking about the notification email of conclusion? If so, after everybody signed a document, an envelope is completed thus an email is sent to all signers with the signed document and a Certificate of Conclusion.
