help with check box on form

Hi, I added 2 check boxes for people to click on and when the forms came back to me none of them were checked. I’m wondering if I added them improperly because I can’t imagine everyone missed that step. Any help with this issue?

That might have been the case. You need to enable Validation on your checkboxes. Also make sure they are part of the same group.
