Bulk uplod name, email and mailing address

Is there a way to customize the bulk upload template to include a mailing address? So that the envelope says:

Name

email

mailing address

@twnsmom1 The envelope name (descriptor) that is set typically is derived from the Email Subject of the first Recipient or from the Email Subject of the Powerform Landing Page.  Both have an ability to use merge fields such as any of the Recipients Names and/or Recipients Email addresses.  However, there is no merge values that can pull data from the actual DocuSign fields on the document, so you are restricted to Recipient Name and/or Recipient Email for the Envelope.
