Bulk Upload format

I would like to bulk upload a list of Names, Email Addresses, Company, Title for each of the 100 recipients. I upgraded for the premium service and I downloaded the bulk CSV sample, but the sample doesnt make sense nor does it show me the fields I want. In addition, the Youtube video that was suggested in another post was private and thus I could not watch it. I look in support for the format of a csv for the upload, however I cannot find anythin on it.

Before you download your sample bulk CSV sample, update your template with all your fields. Get it ready as if you were to send one envelope. After you do this, you can then download your sample CSV. It will now, include all the fields you need to populate.
