Adding parent/guardian signature line to powerform?

Hello - I’m the volunteer coordinator for an animal shelter.   I’m trying to integrate DocuSign as a more efficient way to collect signed waivers from our volunteers.   Many of our volunteers are high school kids in need of community service hours.    

 

I’m not super tech savvy so I’m struggling with figuring out how to add a parent/guardian signature for our under 18 volunteers.   

 

Is this possible within DocuSign?   I’m using powerform so that within the body of the email we send to our applicants, I can include the link as opposed to attached a Word doc to the email.

Thank you for reaching out to the Docusign Community.

I apologize for any inconvenience this might cause you. For this, it would be best to have two Powerforms one for volunteers over 18 and one for those under 18, this way with the underage volunteers you can add a recipient for the parents to sign the envelope.
