My contract must have functioning Check Boxes. I inserted them using the instructions here:
I created a checkbox on the first item, copied it, and created the second checkbox on the second item. I tested the checkboxes and saw that I could click on the first, click on the second, and then unclick the first. So it appeared to be working.
At the signature lines, I inserted text boxes for name, title (at company) and a signature box for the signature.
I filled everything out. The document came back to me with NO CHECKBOXES. Not even checked or unchecked. They simply didn't appear. Just blank spaces.
To repeat: I tested the check boxes and they functioned when I sent them for signature. The signatory checked one of the boxes. But the document was returned to both me and the signatory without any check boxes. Just blank spaces.
