It appears at the bottom of the page and on every page of a document. See attached.

To me, those numbers look like the document name or another field/formula within the Word document, similar to the page numbering next to it. Therefore, this appears to be unrelated to Docusign eSignature.
I would recommend checking that section of the Word template to identify the source and make any necessary changes.
