I requested to not sign a Docusign document. I reviewed the document and it has my signature. I would like to know how I can verify that I signed the Docusign document.

Do you have the Certificate of Completion? This is usually included with the completed envelope. It shows information on who signed/email/time and date/location (if turned on). If you do not have it, you can request it from the sender…

If you had refused to sign via DocuSign the envelope should have been voided.
