Checkboxes not working

I have a form that we have used before but needed to update the document. I put in all the text fields and check boxes. I did a check on the form before sending and was able to type in the fields and choose text boxes. When downloading the form after completing, none of the checked boxes are showing. I tried deleting the boxes and adding them again, uploading the form again and placing the check boxes. You can click and add the check mark but once again when you complete they do not show.
Any thoughts or solutions would be appreciated.

Here are a few things you can try:

  • Re-Add Fields: You've already tried deleting and re-adding the checkboxes and uploading the form again. This is the correct first step.
  • Field Placement: Ensure that the checkboxes are placed directly on the form, not overlapping with other fields or outside the printable area.
  • Flattening Issues: Sometimes, if the original PDF is "flattened" or has form fields that conflict with DocuSign's overlay, fields may not render correctly in the final document.
  • Field Assignment: Make sure the checkboxes are assigned to the correct recipient and are not marked as "optional" if you want them to always appear.
  • Download Source: Are you downloading the document from the DocuSign UI, via email link, or through an integration? Sometimes different methods can yield different results if there's a sync or rendering issue.

Thank you for the suggestions. I ended up linking check boxes together and they seemed to work. Previously I have just added the check-boxes with no issues. I did try deleting and adding them again and it did not work. When I used this form below it was not a PowerForm and this time I had made it one so maybe that caused an issue???? Not sure but I seem to be okay now.
