Our company sends the same template to patients who are admitting to our company. How can I change the formatting for each field I add.

You can change the formatting of fields in a template by editing the field properties after adding the field to the document.
Steps:
1. Open the template you are using.
2. Select the field you added (for example: Text, Date, or Number field).
3. The Field Properties panel will appear on the right side of the page.
4. Go to the Formatting section.
5. From there you can adjust the field settings such as:
Font
Font size
Style
Other field configuration options (for example, making the field required or read-only).
6. Save the template once you finish updating the formatting.
