Envelope report that will allow to capture a custom field values

hello, I am looking for an enhancement to existing reporting. I am looking to create a custom sent envelope report for a specific template and also looking to pull in a specific data label field value for each of these envelopes, like member account number for example, which we capture via text box. Currently there is no way to capture this information in the envelope report.
